Purpose

This questionnaire aims at providing our company with a basis for a preliminary assessment of its suppliers. Confirmation of the data provided may be requested, by means of a test or an audit on the part of our personnel or of a designated organization.
INFORMATION ON SUPPLIERS' PERSONAL DATA PROCESSING
(Italian Decree Law 196/2003)
According to art. 13 of the Italian Decree Law 196/2003 on personal data protection, the data that you have already provided us with or that you intend to provide, are being and shall be processed according to the principles of fairness, lawfulness and transparency, thus protecting your rights.
In relation to this and according to the above Decree, you are informed of the following:
1. Purpose of processing:
Personal data is collected with the aim of meeting requirements connected with the company's economic activity, namely:
- Legal obligations and requirements;
- Establishment of contract relationships;
- Commercial information which you provided in connection
   with promotional activities or commercial offers;
- Market research and surveys with statistical and commercial ends.
2. Processing methods:
- Document/computer-based storage of information;
- Processed data may temporarily not be used; they are kept,
  however, in the event of future business relationships.
3. Data may be forwarded to banks for processing credit collection or payments derived from the accomplishment of a contract; they may also be supplied to the Company's accountants who may use them in the course of their job.
4. Providing one's data is optional; it is nonetheless essential for the following reasons:
- Requirements relating to accounting/social security system/tax purposes;
- Contract obligations;
- Obligations deriving directly or indirectly from legislation or EC rules and regulations;
- Failing to provide data might lead to the non-fulfilment of the
  contract or the termination of the work relationship.
5. Polyline Translations is the holder of the data, in the person of Mr. Quaquero Charles. 
6. Quaquero Charles is responsible for processing both document and computer-based information.
With reference to the above data processing, you are entitled to exercise the rights stated in art. 7 of Italian Decree Law 196/03.
